Gestational diabetes always accompanies an increased maternal and neonatal risk. Insulin is the standard therapy but causes multiple complications. Metformin has less This study aims to assess efficacy of metformin in controlling blood glucose level compared insulin women with gestational .A randomized controlled trial conducted obstetric department Salah Al-Deen general Hospital during period from 1st February - 31st July 2022. Total sample 100 pregnant suffered at (24-28week) age were recruited randomly. The patients divided into two groups: 1-Metformin group (50 patients). 2-Insulin Group Fasting post prandial levels 2 h after breakfast done each visit HbAlc trimester. Follow up was continued till delivery evaluate pregnancy outcome mean significantly higher among (5.8± 0.5) than (5.4 ± 0.8). Preeclampsia lower (14%) (19.6%). hypoglycemia episode (41.3%), Caesarean (58.7%) (37.2%). birth weight (3761.4±470) (3540.9±338). Prematurity found (8.7%) comparison (4.7%) group. non .Metformin effective safe glycemic control diabetes, better outcomes.
